- The Service Coordinator coordinates ongoing eligibility and the Individualized Family Service Plan (IFSP), serves as the family advocate, and is the initial resource for Early Intervention Services under Part C of IDEA.
- The Service Coordinator will conduct in-home assessments, lead the IFSP and teams which include, but are not limited to, the family, medical providers, Eligibility Determination Team, educational providers, and other community-based resources, through the process of problem-solving, negotiation, and resolution of conflicts to provide the best options available for the child and family.
- This position will be done remotely from the Service Coordinator's home office.
- The Service Coordaintor must live in the Madison County area in order to conduct regular in-home assessment visits with each client family.
- The employee must demonstrate competence in the resource and referral process (such as DCS, Head Start, Healthy Families, CSHCS, and Medicaid) to assist families adequately and fully.
