- Jail Health’s mission is to provide comprehensive, compassionate, and respectful care for incarcerated people while mitigating the effects of structural racism and historical injustices as a fully integrated and collaborative member of San Francisco's Public Health Department.
- Our team of eight Nurse Practitioners and eight physicians provides a unique blend of urgent care including infectious disease management and substance withdrawal, as well as full-scope primary care including chronic disease management, health promotion and disease prevention.
- Our patients are 91% male, 41% Black and 27% Latinx, 35% unhoused, 79% under 45 and 1% over 65, and a majority have a substance use disorder and/or a serious mental illness.
- Under the general supervision of the Chief Medical Officer, the Jail Health Services’ Complex Care and Population Health Manager will serve as a clinical leader for healthcare staff and an advocate for patients.
- This physician leader will also oversee population health improvement efforts across Jail Health.
