Overview:
<p style="margin: 0px;"><span style="font-family: 'Arial','sans-serif'; font-size: 12pt;">Summary:  The Service Coordinator assumes responsibility for coordinating programs and services to help residents maintain a good quality of life and age in place. Assisting in assessing and identifying resident and family needs and outside resources to meet those needs. Areas of assistance may include physical, mental, psychological and/or social as it relates to aging and moving in/moving out.</span></p>

Responsibilities:
<ol><li><div style="margin: 0in 0in 0pt;">Responsible for networking with community organizations, and seeking affordable and new services to offer residents.</div></li><li><div style="margin: 0in 0in 0pt;">Initiates and completes psychosocial assessments, interviewing patients, families and others in order to evaluate current condition and identify current or potential adjustments.</div></li><li><div style="margin: 0in 0in 0pt;">Provides general case management (and referral services) and maintains complete and updated resident files. Monitor and manage supportive services where necessary.</div></li><li><div style="margin: 0in 0in 0pt;">Service Coordinator meets requirements outlined in Quality Assurance Review Process and Semi-Annual File Review Process (Reviews). The Reviews capture resident assessments, resident programs, policy compliance, and Education and Wellness Programs.</div></li><li><div style="margin: 0in 0in 0pt;">Complies with all Federal and State confidentiality regulations including but not limited to HIPAA.</div></li><li><div style="margin: 0in 0in 0pt;">Coordinates services for optimal living (i.e. aging in place and wellness philosophies).</div></li><li><div style="margin: 0in 0in 0pt;">Utilize appropriate documentation system to track all work done with residents. Completes all required paperwork with all residents who want to utilize the Service Coordination program.</div></li><li><div style="margin: 0in 0in 0pt;">Completes Care Plans/Quarterly Monitoring with residents needing referrals and follows up on all services at and after implementation.</div></li><li><div style="margin: 0in 0in 0pt;">Submits Semi-Annual Performance Reports to HUD in order to maintain funding for the Service Coordination program at the property.</div></li><li><div style="margin: 0in 0in 0pt;">Provides Administration with regular status reports, including preparation and possible presentation to the board.</div></li><li><div style="margin: 0in 0in 0pt;">Identifies and reports resident violations to appropriate authorities.</div></li><li><div style="margin: 0in 0in 0pt;">Engage all residents to identify areas of need, and make necessary referrals. Organize and promote resident involvement in on and off site programs.</div></li><li><div style="margin: 0in 0in 0pt;">Assists in the coordination of home care services to enable independent living and aging in place. Including development of a Resource Directory that include a list of service providers. Continually educates residents (and staff) on a variety of matters including service availability, application procedures, resident rights, health/safety/fitness, issues relating to aging etc.  Sponsor educational events related to health care, agency support, life skills and referral sources. Qualifications:
