- The Diabetes Nurse Educator is a professional nurse, who independently directs, initiates and implements patient diabetes teaching and learning with a body of expertise knowledge and advanced skills, and ultimately inducing positive outcome to empower and promote behavior changes necessary for optimal health, psychological adaptation and quality of life for the patient with a chronic disease, Diabetes Mellitus.
- Utilizing a multidisciplinary approach, and working closely with nursing staff, physicians, dieticians, case managers, physical therapists, the diabetes nurse educator will assess, plan, implement, document, and evaluate educational instructions and outcomes given in a self-management directed program of diabetes education.
- Certified Diabetes Educator (CDE) through the National Certification Board for Diabetes Educators or Board Certified- Advanced Diabetes Management (BC-ADM) through the American Association of Diabetes Educators
