- Mental Health clinician to provide care coordination and outpatient treatment for patients being discharged from the inpatient psychiatric units.

- Will work closely with the inpatient staff and will provide bridge visits prior to patient’s discharge, develop crisis plans, provide short term individual and group therapy post-discharge, coordinate with collateral agencies, assist in disposition planning, and provide crisis intervention and triage.
- Will utilize clinical skills to create a positive working relationship with patients and support network, in order to improve aftercare compliance.
