This continuum is designed to meet the complex needs of each individual and creates a continuum of services that are able to meet the needs of children, adults, and families through a continuum of services utilizing an integrated care model. Title: Clinician Salary: Starting at $70000 / annually depending upon education, experience, and licensure. Responsibilities: <ul> <li>Serve as a liaison between the program and the family </li><li>Provide on-site family therapy </li><li>Interact with the family and the treatment team during the referral and admission process </li><li>Complete a Biopsychosocial (BPS) assessment, intake, and other assessments as required </li><li>Complete IMDS Strengths and Needs Assessment (within the first 24 hours and as needed in conjunction/coordination with CFT) </li><li>Assist with program admissions or inpatient hospital admissions/transitions as necessary </li><li>Develop cross-systems crisis prevention and intervention plans, comprehensive treatment and transition plans (within the first week and update as needed) </li><li>Complete the initial safety and soothing plan development, documentation, and consultation (within the first 48 hours of admission) </li><li>Complete the initial safety and soothing plan debriefing with family and youth (within the first 48 hours of admission) </li><li>Develop and oversee the implementation of the Behavioral Assistant ISP, modify as needed based on frequent evaluation of implementation and progress </li><li>Provide direct supervision of the Behavioral Assistants as required by Medicaid Regulations and Board standards. </li><li>Provide on-site individual therapy as indicated in the youths treatment plan with EBP approaches and in coordination with consultant adjunct therapists or wellness service providers </li><li>Implement interventions and activities based on individual needs and ongoing assessment </li><li>Attend and facilitate treatment team meetings </li><li>Share on-call responsibilities to ensure a timely telephonic response to crises within one hour of the call and face-to-face within 24 hours of the call </li><li>Provide coordinated support with program staff and participate as part of the clinical team </li><li>Provide daily clinical triage support </li><li>Provide systemic consultation </li><li>Prepare agendas and document outcomes for in-home sessions </li><li>Ensure the coordination of treatment team meetings </li></ul>Qualifications: <ul> <li>Independent licensure in NJ; LPC, LCSW </li><li>One-year experience working with youth within family systems and 1 year experience working with people who have mental health and/or co-occurring behavioral health needs including complex trauma </li></ul> Assertive Community Treatment Services for Youth (Youth ACT) The program operates throughout Cape May and Atlantic County and delivers a multidisciplinary mobile team to provide intensive and immediate access to treatment services in the community. The program works with youth ages 5-20 who present with significant mental health needs, challenging behaviors, and/or are at risk of psychiatric hospitalization or out of home care.
