- As a member of a multidisciplinary team, the Time Limited Rental Assistance Program Case Manager is responsible for providing a full array of counseling, case management, advocacy, and group work services to young adults ages 18-24 experiencing homelessness who are preparing to transition to permanent housing in the community, while gaining job training, employment, and educational and life-skills competencies.
- Oversee the diverse case management needs of approximately 10 young people experiencing homelessness funded through DYCD/RHY and HUD.
- Using evidence based tools, assess history of housing and homelessness, risks faced, socialization, daily functioning, and health and wellness.
- Responsible for providing aftercare services on a quarterly basis to all program participants who have managed to complete the full three years of the program.
- Promote and model the 7 Sanctuary Model commitments and the SELF (Safety, Emotional Management, Loss, Future) Model into daily practice to create a non-violent and trauma informed culture
