- The Behavioral Health Therapist will provide case management and supportive services for children and families, adults, older adults, and probation youth.
- Maintain documentation per Santa Clara County Behavioral Health Division and Catholic Charities standards in the AWARDS data system.
- Under the RISE program, work closely with the Probation Officers and juvenile hall to support youth on probation.
- Work closely with psychiatrists, primary care physicians, vocational programs, therapists, legal representatives/guardians, Veteran's Administration staff, Department of Mental Health / Drugs and Alcohol staff, parole and probation officers, judges, police, and other community services/organizations to assure the individual's access to services and the service delivery systems.
