You will be a full-time employee at UTHealth of Houston and work as part of the team with the Childrens memorial hermann hospital.
This applicant will have access to all of the evidence based health education provided by UT Health as a system.
Work in coordination with physician, nurses and social workers to interview patients that have been enrolled or are being asked to enroll in a medical home.
This employee will be responsible for the ongoing design build, testing, validation and support of any and all assigned modules across the health care system.
